Sinai In-Home Care at Cedar Sinai Park is administered by a local, licensed, non-profit, full-service agency for those in Multnomah, Washington, and Clackamas counties. Our dedicated team supports elders and people with disabilities who want to remain in the home they love, but need trusted assistance to do so.

With four levels of care, our exceptional in-home care program offers support to you and your loved ones. Our services range from light housekeeping, meal prep, assistance with activities of daily living, medication management, memory care support, and help with many other special needs. Customized Care Plans:

All services begin with a thorough in-person assessment with you and your family. We’ll explore the different ways that we can help and develop a care plan that is tailored to meet your needs. This care plan guides our staff and provides you with the best possible support for your situation.

Caring Staff that Serve with Love, Honor, and Respect:

Our staff are hands-on and collaborate with family and other members of the care team about ongoing needs, progress, and changes. All care is supervised by our Oregon licensed and registered Nurse Case Manager

LEVELS OF CARE

Companion Care

Short or longer-term visits in your home for companionship, safety, and supervision with limited assistance. We are a friend and an extra hand when you need it.

Homemaker Care

Assistance with your daily activities. Includes Compassion Care benefits, in addition to help with appointments, housekeeping, cooking, meal preparation, general errands, and more.

Personal Care

A more hands-on approach to daily care. Includes the benefits of Companion and Homemaker Care, while also addressing physical needs such as bathing, dressing, and general mobility. A steady hand is part of our reliable care.

Complex Care

Advanced care for those who may benefit from greater oversight. Services include all care offered, plus diligent monitoring, medication administration, nursing services, and additional personal assistance with addressing cognitive needs.

OUR APPROACH

From inquiry and assessment to the end of services our approach is designed to provide, clear communication and support every step of the way.

Assessment

This initial phase includes meeting and introductions. We will discuss your care preferences and desired outcomes. We listen carefully to reach a better understanding of your physical, mental, and emotional needs. With this information we determine the appropriate level of care and draft an individualized care plan.

Care Plan

The care plan describes our understanding of your physical, cognitive, and functional limitations; states your health and wellness, and defines specific tasks and preventative actions (as necessary). It describes medical equipment and supplies that are used in the home, explains special nutritional needs, and food or medication allergies. Additionally, it outlines the types and frequency of services to be provided at each visit. Our individualized care plans can be adjusted with the changing needs of each client.

Monitoring

Home visits are conducted by our qualified staff to ensure excellent care. The first visit is held a week after the first service date and not later than 30 days; follow-up visits take place quarterly. If medication administration is required, our registered nurse will conduct cognitive self-administration evaluations on a quarterly basis.

Conclusion

When our services are no longer needed, we will close the account after confirmation from you. This may happen with a phone call, in-person meeting, or in writing.

Sinai In-Home Care policies and procedures are in accordance with the Oregon Administrative Rules establishing the standards for licensure of in-home care agencies operating in Oregon. All with the dignity and respect you would expect from the Cedar Sinai Park family.
